The Hospice Clinical Intake LVN coordinates services, to facilitate a smooth transition from medical facility to Hospice Care. This position promotes continuity of care between medical facility staff, physician, and family members. They will be responsible for educating and assessing patients referred to hospice to ensure hospice criteria is met and informed decisions regarding participation in hospice are made. Lastly, this position will facilitate prompt and appropriate delivery of Hospice patient care in accordance with Medicare Conditions of Participation, in Orange, Long Beach and surrounding Los Angeles areas.
Essential Job Outcomes
Assist & coordinates prospective patient/family in making informed decisions regarding Hospice care during the pre-admission process by presenting hospice benefits/services.
Performs comprehensive nursing assessment and evaluates for appropriateness of hospice services prior to admission.
Actively collaborates and communicates with hospital case managers and physicians regarding hospice referrals.
Educate patient, family, staff, physicians regarding hospice coverage and services available after discharge to insure appropriate level of care and utilization of services.
Facilitates hospital discharge by coordinating transportation, ordering medications, and ordering appropriate medical equipment.
Accepts and coordinates agency referrals and assesses patients to determine level of care and admission criteria appropriateness, documents pertinent information clearly and accurately according to hospice guidelines.
Documentation meets timeliness standards as set by the agency.
Meets productivity standards as set by the agency.
Actively participate in Hospice activities including in-services, performance improvement and appropriate committees.

Primary focus is on patient and family relations by guiding family and loved ones throughout the hospice journey.
Identify potential donor opportunities and coordinate effort with SB Foundation.
Coordinate and ensure post logistic needs are consistently identified and met.
Set family and loved one’s expectations and guide through the dying process.
Maintain relationships to further identify and serve future potential needs.
Serve as a single point of contact for hospice patient and family/loved ones.
Ensure the MemorialCare Hospice experience is consistent and exceeds expectations.
