Geriatric Advanced Practice Provider for Perioperative Care
The Perioperative Optimization of Senior Health (POSH) Program Advanced Practice Provider (APP) provides direct patient care services and performs medical acts as an APP in caring for older adults undergoing elective surgery at Duke University Hospital under the supervision of Geriatric Medicine Physician Faculty. The APP performs physical exams, collects and documents data, conducts diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, and provides direct patient care services. The POSH APP's primary responsibility will be to provide postoperative consultation and follow up to optimize the patient experience and surgical outcomes by focusing on strategies to prevent delirium, functional decline, and falls and assures optimal nutrition, pain management and medication use. The POSH APP will collaborate with the geriatrics team to assure optimal and safe transition to the post-acute environment. The POSH APP will also practice in the preoperative clinic one session per week and as coverage needs dictate. In both settings, the POSH APP will be supervised by geriatric medicine attending physicians. The POSH APP will collaborate with the Duke NICHE steering committee. As such the APP will contribute to the development of Duke NICHE's annual strategic plan and activities to ensure the complex needs of older adults are met and the plan is in alignment with health system priorities. The POSH APP contributes to excellence in patient care, research, teaching, and provides leadership in the organization.
Completion of an accredited MSN or DNP Nurse Practitioner program with national certification as an acute care, family or adult NP from a national certifying board
Completion of an accredited Masters Degree Physician Assistant program with national certification from NCCPA.
Prior experience as a Nurse Practitioner or Physician Assistant in a comparable clinical setting strongly preferred.

Able to clearly articulate scope of practice, and practices within those guidelines as an APP in the state of North Carolina and within the Duke University Health System.
Able to independently seek out resources and work collaboratively to solve complex problems
Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others.
Able to use sensory and cognitive functions to process and prioritize information, perform health assessments, treatment, and follow-up.
Able to use fine motor skills
Able to record activities, document assessments & interventions; prepare reports and presentations;
Able to use computer and learn new software programs
Able to provide leadership in clinical area of expertise and in meeting organizational goals
Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations
Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds
Able to remain focused and organized
Able to work collaboratively with all levels of personnel
Primary hours will be Monday-Friday, 8am-5pm, and other hours as needed to fulfill the responsibilities of the position. This schedule may be changed based on service needs and as determined by the Center for Advanced Practice in consultation with clinical leadership.
