The PGEC (Purchased Geriatrics & Extended Care) RN (Registered Nurse) assesses and coordinates patient care through the following activities: collaborating/consulting with interdisciplinary team members, assists in discharge planning, acting as a resource liaison for exploring extended care options, providing education regarding referral processes, and facilitates communication between healthcare providers and community resources. Basic Requirements: English Language Proficiency.
