Nurse Practitioner Palliative Care
Join Providence Hospice of Snohomish County as full-time Palliative Care nurse practitioner. The nurse practitioner will be the main provider for the community-based Palliative Care program for adult patients. The program was initiated in April 2021 and has a current census of 55. In this position, the NP will make in-person home visits with oversight from the medical director and program director. Follow-up visits will be made according to need, with a cadence of every 3 to 12 weeks. The work will involve only out-patient (home based) palliative care, and the NP is expected to collaborate with the patient?s PCP and specialists.
