Assist with training and coordinating clinical review and quality assurance. Lead and review the appropriateness of clinical care and documentation practices related to patients/residents. Review and evaluate quality data to determine areas needing improvement. Review patient satisfaction to ensure excellent quality of care. Chart audit oversight and performance improvement planning for all programs. Oversight of Midas and infections to track and develop PI plans. Assist with EMR best practices and policy review and maintenance.
Qualifications:
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
Bachelor Degree Required Health related clinical degree
Masters Preferred (Health related clinical degree; Masters or doctorate preferred )
3 year of recent home health and/or hospice experience required, 5 years preferred. Requires the employee demonstrate competency in the provisioning of individualized care of patients/residents, comprehensive understanding and extensive clinical experience with documentation of the home health and/or hospice patient.

Valid CO Driver?s License.
BLS
Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally.
This hospice role works Monday through Friday 8:30am - 5:00pm. You can expect to administrate call rotation for 7 consecutive days approximately (once a month and as needed).
