Advanced Primary Care (APC) - Nurse Practitioner
POSITION SUMMARYThe APC Nurse Practitioner (APCNP) functions in an Advanced Practice Role as a clinical expert and independently performs and documents a complete physical examination for acute and complex chronically ill patients. The APCNP administers medical care to assigned population. The APCNP administers preventive care and chronic illness surveillance. The APCNP’s responsibilities include assessment, diagnoses and management of health-illness needs. The primary goal is to deliver Age Friendly Health Systems by focusing on geriatric principles, including the 4M’s (What Matters, Medication, Mentation and Mobility) using evidence-based practices in the care for older adults.
