The Eastern Oklahoma VA Health Care System, Social Work Service, is currently recruiting one Senior Social Worker - The Patient Aligned Care Team (PACT) in Claremore, OK. To qualify for this position, you must meet the requirements of this position by the closing date of this announcement. BASIC REQUIREMENTS Citizenship. Be a citizen of the United States. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). Grade Determination for the GS-12 Senior Social Worker Licensure/Certification. Individuals assigned as Senior Social Workers must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Education and Experience. The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. The incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans and social determinants of health. Social Work responsibilities include assessment of psychosocial issues that cause distress, often impacting health and creating barriers to care. These social determinants of health can include adjustment to or management of medical condition(s), untreated or undertreated mental health issues, substance use disorders, economic instability, food insecurity, legal issues, inadequate housing or transportation, and barriers to access to care. Referrals can originate from any member of the interdisciplinary team, from Veterans and/or their families who call or walk-in for assistance, and from community professionals. The incumbent must complete thorough assessments to clarify the presenting issue(s), possible underlying causes of the presenting problem(s), interpersonal and/or environmental factors impacting the Veteran, and the effects of these on the Veteran's well-being and overall functional status. The Social Worker will work with the Veteran and/or Caregiver on appropriate interventions and work toward setting shared goals for increasing level of functioning, health status, well-being, and overall quality of life utilizing advanced practice skills, interventions, and referrals to internal and external supportive and treatment services. The Social Worker will maintain appropriate ethical practice and work to promote a patient-centered approach to interventions and care while supporting the autonomy of the individual. The incumbent will help the Veteran and family to understand the pros and cons of possible short-term and long-term solutions, encouraging them to make positive and lasting changes to reduce stressors and increase overall health and well-being. The Social Worker will coordinate with VA staff and community agencies to assist in problem solving as needed. The incumbent: (1)Must have a high level of skill and expertise to establish and maintain effective therapeutic relationships with highly complex Veterans in the Primary Care setting. The incumbent must work independently with Veterans and families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, legal and psychosocial issues. Effective Social Work in the Patient Aligned Care Team context often occurs over a series of several visits. (2)Must have knowledge of an interdisciplinary team approach and learn the role of each health and allied healthcare team member as well as an understanding of the role of the Social Worker in the PACT model. (3)Must independently complete comprehensive psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. The incumbent must assess functional acuity of the patient and provide appropriate interventions. (4)Must actively participate as a member of the interdisciplinary treatment team and actively participate through collaboration with Veterans and family as well as interdisciplinary treatment team members in the development and implementation of treatment goals and interventions. (5)Must possess a working knowledge and experience in use of medical and mental health terminology and diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, substance use disorders, and common medications and their effects/side effects. (6)Must facilitate action for community placements through collaboration with Veterans and their families as well as interdisciplinary treatment team members to ensure appropriate community placements are completed in a timely manner. Community placements can involve but are not limited to referrals to group and family care homes, assisted living facilities, adult day health care programs, state Veterans homes, VA contracted nursing homes, Community Living Centers, residential care homes, and inpatient or outpatient hospice services.
