The Home Based Primary Care (HBPC) RN Nurse I/Level 3 is a case manager for the Veterans enrolled in their panel. HBPC provides primary care services for Veterans who have a number of chronic medical conditions requiring longitudinal case management by an interdisciplinary team in the home environment. Patient populations include male and female, adult and elderly, frail patients. This is a intermittent position, on call with a varied/intermittent work schedule. Shifts will vary. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
