The Geriatrics - Community Living Center (CLC) physician will provide direct patient care and supervise medical house staff, medical students, and/or geriatrics fellows. In addition to their role as clinician, the CLC physician will have major administrative responsibilities for leadership and direction of CLC. Administrative responsibilities include program development and evaluation, as well as effective interface with the Medical Center.
The ideal candidate will be board certified/eligible by American Board of Medical Specialties (ABMS) or the American Osteopathic Association (AOA) in Geriatric Medicine. In addition to providing primary care for a panel of Veterans within the CLC, the selectee will be a clinician educator, who will assist with oversight of an increasing number of learners anticipated in the coming years with the growth of our geriatrics service line.
Practice Highlights:
The incumbent evaluates and manages adults who have single or multiple disorders in all the subspecialties in the broad field of internal medicine and specifically geriatric medicine. The incumbent must be capable of working in interdisciplinary teams to provide comprehensive patient care.
Actively participate in defining and accomplishing the mission and goals of the HJH CLC.
Assist in developing short- and long-term strategic plans for the HJH CLC.
Collaborate with the CLC management to advise the GEC administration regarding adequacy and appropriateness of scope of service for CLC residents, CLC staffing models, and issues related to cultural transformation.
Function as advisor to the ACOS for Geriatrics and Extended Care on all medical care issues of the HJH CLC.
Serve as an advocate for the elderly patient by reviewing current programs and scope of services, identifying unmet patient needs, and as resources allow, recommend changes in or additions to these services.
Be an integral part of the planning process in the hiring of new clinical staff and in the discussion of leadership changes in the ancillary services of the HJH CLC.
Be responsible for the organization and coordination of provider services within the GEC service line.
Assist with the organization and coordination of services provided by other health care professionals as they relate to patient care.
Provide direct patient/resident care including admission H&P, all day-to-day aspects of patient care, discharge summary, participation in discharge planning and any family conferences.
Develop rules and regulations for all the providers who attend residents of the CLC.
Ensures performance and quality of care through mortality and morbidity reviews, peer reviews, and occurrence screens.
Assume administrative responsibility for physician schedules, timecards, leave use and performance evaluations.
Ensure 24-hour daily physician coverage by arranging for routine and unscheduled medical coverage in all CLC units.
Monitor physician practices and responsibilities and make credentialing and privileging recommendations to the Chief of Staff and Professional Standards Board.
Lead medical staff meetings and in-services.
Ensure that medical staff complete mandatory VA annual training and in-services and sign-off on all clinical documentation in a timely fashion.
Help to develop emergency treatment procedures for residents/patients.
Participate in the creation of policies, procedures, and guidelines to assure provision of high-quality comprehensive services, including procedures for emergency treatment of residents in the CLC.
Evaluate the cost effectiveness of the care delivered.
Utilize an interdisciplinary approach to medical care.
Collaborate with the Admissions Coordinator to ensure safe and appropriate CLC admissions, maximize the utilization of CLC beds, and maintain transparency in the admission process.
Support and direct the teaching mission at the CLC in coordination with the Medical Facility and any outside teaching organizations.
Work Schedule: Monday through Friday; 7:30 a.m. - 4:00 p.m.
