Hospice of Chattanooga, a part of Care Hospice, is GROWING! We are hiring a PRN Compassionate Licensed Clinical Social Worker (LCSW) to provide palliative care social services in and around the Chattanooga, TN Area! This position has the opportunity to work remotely! However, this position would still require you to do occasional face-to-face visits which means that any candidates MUST be local.
Our Licensed Clinical Social Workers (LCSW) are responsible for the delivery of various social work services to palliative patients and their families. Our Social Workers (LCSW) provide social service counseling and guidance to facilitate the maximum functioning and coping capacity to each patient and caregiver/family member.

Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the RN Case Manager and other members of staff including the interdisciplinary group.
Assess the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, including funeral arrangements if needed.
Supervises the implementation of an individualized social work plan of care and notified the Bereavement Coordinator of bereavement assessment needs.
Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses and symptoms of the terminal illness.
Counsels patient and family/caregivers as needed regarding to stress and other identified coping difficulties.
Qualifications
Masters of Social Work from an accredited school of social work
Holds current unencumbered license of Social Work in the state practicing in.
Minimum one (1) year experience as a social worker in long term care or medical surgical/acute care setting.
One (1) year of hospice experience preferred but not required.
Must be able to work flexible hours and travel between offices, facilities, etc.
Must be computer proficient in typing and various programs, including background in EMR.
Must have ability to handle stress associated with ministering to patients/residents and their loved ones who experience pain, suffering, death and grief.
Possess and maintains current CPR certification.
CMS requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully vaccinated, or have a documented religious or medical exemption. This important rule is required for all healthcare organizations that receive federal funds through Medicare or Medicaid

Our team of physicians, nurses, certified nursing assistants, social wor...kers, grief counselors, and volunteers provide a full range of care to maximize comfort. We also work with local hospitals and senior living facilities to ensure our patients who require a higher level of care have that option.
