Registered Nurses in the Home Care Services department at UC Davis Medical Center provide care to adult and pediatric patients with a variety of health care needs including post-operative, medical, palliative and end-of-life care. Patient conditions include both acute and chronic conditions spanning across many care specialty areas and age ranges. This position will be required to provide direct patient care, case management and associated activities to pediatric patients, including pediatric palliative care and hospice patients and stable adult patients.
The Home Health RN 2 identifies and implements nursing interventions and evaluates results for a wide variety of patient populations, directs and acts as resource to other members of the nursing team as assigned and promotes compliance and support for policies. The primary responsibilities for this position consist of providing total, comprehensive, continuous and patient centered care which includes: assessing, documenting, planning, implementing, teaching, developing, and evaluating a comprehensive nursing plan of care; delivering direct patient care; ensuring patient satisfaction; training and delegating duties; and adhering to University policies and procedures as well as federal, state and local regulations.
Provides direct patient care to a diversified patient population. Applies knowledge, experience and judgment to determine the importance of a situation, set priorities, and use abstract thought to evaluate clinical situations. Participates in patient and staff education. Demonstrates leadership skills. Demonstrates accountability for own practices as defined by the Nurse Practice Act.
Required Qualifications
Two (2) years’ experience as a Home Health RN Case Manager in a Medicare-Certified home health agency
Pediatrics Experience Required
Valid California driver’s license is required
American Heart Association (AHA) BLS CPR Certification or UCDH-issued CPR Certification
Valid California Registered Nurse License without restriction
Verification of graduation from an accredited school of nursing
Ability to read, write, speak, and understand the English language.
Ability to utilize medical terminology appropriately.
Knowledge of current trends in nursing.

Preferred Qualifications
Certificate for OASIS Specialist – Clinical (COS-C)
Certified Hospice and Palliative Nurse (CHPN)
Bachelor’s of Science in Nursing preferred.
At least two (2) years of recent RN experience in inpatient, acute care setting
Special Requirements
Employees must participate in the DMV Pull Notice System
Has mandatory reporting responsibilities for domestic violence, child abuse and elder/dependent abuse.
Ability to safely lift 25 pounds and assist in moving patients, and to stand on feet over extended periods.
Must be able to speak well enough to communicate over the telephone and hear well enough to take vital signs.
Must be able to travel to patient’s homes.
Some restricted vacation due to program needs is possible.
Must work evening, on-call, weekends, holidays and overtime as defined by the collective bargaining agreements. Will act as relief night charge nurse on occasion.
