Responsible for supervision and operational direction of home health and or hospice operations.Provides supervision of the clinical care and non-clinical services delivered to patients with twenty-four-hour accountability for patient care and operations of the home health and or hospice agency. Performs duties in accordance with applicable state, federal and TJC regulations. Assumes responsibility to achieve performance improvement objectives, financial outcomes, and clinical outcomes to assure Centura meets the needs and expectations of its patients, families and referral sources. Assures collaboration across the enterprise of Centura and is a service excellence champion to ensure the agencies are constantly striving to be the provider of choice.What You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following: * Associates Degree PT, OT and MSW also accepted (Required)* Bachelor required within 2 years of hire or transfer for Nurse candidates only.* 1 year of recent home health/hospice/palliative care. Requires the employee to demonstrate competency in the provision of individualized care of patients based on the assessed age and developmental needs* BLS* RN, PT, OT, or MSW License* Valid Driver's License Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly.
