POSITION SUMMARY: Responsible for the coordination and integration of optimal care transitions for all patients. Provides Care Coordination services to patients based on person-centered acuity and functions as a leader for care coordination in an interdisciplinary team setting. Contributes to a cooperative and accountable working relationship with all members of the Senior Care Program towards the goal of providing the highest level of customer satisfaction. Promotes optimal person-centered care that supports and empowers individuals, respects individual choices and facilitates needs of Geriatric patients.

MINIMUM QUALIFICATIONS:

EDUCATION: RN, BSN in Nursing, preferred.

CERTIFICATION/LICENSES: Current New Mexico Nursing License

SKILLS:

• Considerable knowledge of health care and social management principles, practices and procedures
• Ability to coordinate various functions and activities for maximum cooperation and integration of services among providers and persons receiving care
• Ability to instruct and advise staff, and to receive and effectively react to day-to-day problems
• Ability to utilize strong communication skills, both written and oral, and effectively demonstrate a supportive and compassionate manner to customers
• Ability to understand and implement sensitivity and culture of care measures appropriate for a diverse population
• Ability to adapt to a variety of clinical situations and to support all levels of patient care services
• Ability to work within various levels of the organization including hospital, clinic, and outpatient settings.

EXPERIENCE: Minimum of two years of experience in a health care setting. Bilingual English/Spanish preferred.

NATURE OF SUPERVISION:

-Responsible to: Manager of Population Health, Director of Patient Care Services

ENVIRONMENT:

-Bloodborne pathogen: Multiple settings and variable unpredictable situations including inpatient, outpatient,

Specialty and primary care settings.

PHYSICAL REQUIREMENTS: Ability to move around settings of care. Requires close work, good vision, dexterity to write as well as to use computer keyboard. Ability to communicate effectively in public venues. Ability to handle stressful situations.
