Incumbent is a Clinical Social Worker assigned to the Marion VA Health Care System and associated Health Care Centers and Community Based Outpatient Clinics (CBOCs). Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: Social Worker (Program Coordinator), GS-12-(1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. DEMONSTRATED knowledge, Skills and Abilities at the GS-11 Social Work Level are defined as: (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. (3) GS-12 Program Coordinator demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Referrals typically originate from members of the interdisciplinary team, from Veterans and/or their family members/significant others coming as walk-ins to seek assistance, and from community professionals. Consultation with members of the outpatient interdisciplinary team is provided in order to coordinate a collaborative effort to meet the agreed upon goals for the Veteran's treatment needs and include liaison with community professionals regarding needed services for Veterans, their families, and/or caregivers. The position functions as the Advance Care Planning via Group Visits (ACP-GV's) diffusion initiative site group facilitator. The position will be responsible for coordinating and facilitating ACP-GVs at sites and contacting ACP-GV attendees by telephone within required time frame for follow-up. Duties include preparation of local policies to conform with VISN and National directives and management of the daily operation of the program. The position is responsible for tracking and monitoring the ACP-GV's goal areas. The position will be responsible for attending National ACP-GV's calls and trainings, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for ACP-GV facilitators and affiliated staff. The position will be responsible for educating all Marion VAMC staff on Advance Care Planning policies, procedures and changes to practice. This includes the Life Sustaining Treatment Initiative and ongoing End of Life practices. The position will focus on ensuring all Marion VAMC Veterans have Advanced Care Plans and Life Sustaining Treatment Decision Plans in place. The incumbent will be viewed as the service and a facility Subject Matter Expert in regards to the clinical, ethical and patient-centered approach to ensure education and consultation regarding treatment planning and end of life care. The incumbent must complete thorough assessments of patients presenting with advanced illness and end of life needs to determine the presenting problem and to evaluate in -home factors impacting the problem to determine obstacles to treatment and/or follow through with recommended treatments. Working with various patient populations include, but are not limited to, homeless Veterans, Hospice services, Community Living Center, Home Health services, financial resources, VA benefits, and transportation services. The incumbent is capable of practicing at the advanced level and has specialized knowledge of practice typically related to the acutely ill population. Incumbent should have at least 2 years in an area of specialized social work practice and be licensed or hold certification in a state at the independent practice level.
