Assessment, planning, etc. of case management in HBPC program. Consultation on psychosocial treatment of patients in HBPC program rendering professional opinions based on experience/expertise & effective social work skills. Coordinates activities including outreach, psychosocial assessments, treatment planning, consultation/education to staff & community-based service providers, collection & maintenance of data, program planning evaluation & modification & supervision of students as assigned.
