The Social Worker is a member if the interdisciplinary team whose primary function is to provide diagnostic assessments of the psychosocial and economic needs of the patient/family unit, define social service goals for alleviating these needs and help provide both the counseling and case management needed to meet these goals. Our Per Diem Social Worker, provide coverage on an as needed basis to ensure patient care needs are being met. 

This position is on a Per Diem / as needed to help provide coverage and visit assistance to our patients and their families. Our PRN Social Worker must be able to travel throughout our hospice service area to conduct admissions, follow up and routine visits. Our office is located at 1201 S. Grand Blvd. St. Louis, MO 63104.

Primary Responsibilities:

• Participating in care conferences and working in interdisciplinary teams
• Conducting visits and providing coverage for vacations, PTO days and/or leaves of absences
• Participating in on call on a rotating basis (as needed only)
• Respond to Social work consult within 24 hours

Required Qualifications:

• Master's Degree (or higher) clinician in Social Work (MSW)
• Ability to work two 8-hour shifts per month
• Ability to work onsite at 1201 S. Grand Blvd. St. Louis, MO 63104
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance. Preferred Qualifications:

• Active / unrestricted LCSW or LMSW in the state of the hospice
