What You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:  * Master of Social Work (MSW).* 6 months home care/hospice experience preferred.Two years post Master's experience in health care or related field preferred.* Licensed Clinical Social Worker (LCSW) preferred.* Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED.* Current Driver's license Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly This roleis a day time role (8:30am - 5:00pm).  This is primarily a weekend position, but we could offer some weekday hours if desired.  There is a requirement to cover 1 holiday a year.  The ideal candidate would have some hospital experience. Your Connected Community:When people and places align, incredible things happen.
