Providence Medical Group at Home is seeking a part-time or full-time nurse practitioner to join a skilled interdisciplinary team dedicated to providing home-based primary care to patients in a community setting. In this role, you will provide consultations for acute conditions, geriatric assessments, and transitions of care, in addition to primary care for a limited group of complex, permanently home-bound patients who are not best served in a PMG clinic setting.
