Home Based primary Care provides comprehensive, interdisciplinary, primary care in the home of Veterans with complex medical, social and cognitive disorders for whom routine clinic-based care is not effective. The primary focus of HBPC is to serve Veterans during the months and years before death, providing primary are, palliative care, rehabilitation, disease management and coordination of care services. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
