The Community Home Health Nurse position is responsible for coordinating skilled and non-skilled services for the Community Health Program utilizing community-based agencies that are under the MISSION Act and/or Provider Agreements which includes: Skilled Care (SN, PT, OT, Speech Therapy, Wound Care, IV Therapy, and Medication Management) Home Hospice, Home Health Aide, Homemaker, Respite and Bowel and Bladder care. The Community Health Registered Nurse functions as a liaison between the VA Nebraska Western-Iowa Healthcare System and community skilled/non-skilled agencies to facilitate care coordination and follow-up services among community vendors, PACT, CLC, and inpatient treatment teams.
