Board Certified or Board Eligible - Hospice Palliative / Geriatrics
In this role the provider will be required to work in a multitude of services including Home Palliative Care, Home Hospice, In-patient Palliative Med Consults, Out-patient Palliative Medicine clinic, Skilled Nursing Facility and Advanced Medical Care at Home.
