The Coordinator for the Critical Care Family Assistance Program in the hospital intensive care units which includes the SICU, MICU and CICU. Works closely with the Palliative Care Coord to provide services as deemed necessary for families of Veterans in the Palliative Care Unit. Ensures administrative responsibility for the operations of the program and manages daily operations. Develops budget for the CCFAP and is responsible for control points and allocation of funds decision-making Develops and administers family surveys. Re·cords and tracks data for preparation of goal setting and action plans. Prepares program budget and oversees program expenditures. Serves as control point official for the Family Care Program. Chairs the CCF AP multidisciplinary leadership committee which focuses on meeting the needs of the veteran and their family. Coordinates with various VA staff, VA and community organizations to accomplish to goal of the CCFAP. Practices at an advanced level and independently gathers relevant information to guide practice, including community Social Work standards. Uses initiative and resourcefulness in deviating from traditional methods or trends and patterns to 'develop new methods, criteria, and proposed new policies. When assigned a social work student, incumbent will supervise training activities of the student and will adhere to VHA Handbook 1400.0 Supervision of Associated Health Trainees and VISN Documentation Policy ION2-l l 5-08. Serves as a mentor to other Social Workers, particularly those at the entry level. Provides clinical supervision for unlicensed Social Workers and for Social Work associates. Is viewed by Social Workers and interdisciplinary team members as a senior clinician with advanced practice skills and expertise. Provides consultation to other Social Workers and interdisciplinary team members as needed. Incumbent is responsible for development and maintenance of professional standards of practice and education others about_ them. Provides training to students, residents, interns, and fellows from other disciplines. Instructs them on psychosocial treatment and on the services available from Social Workers. Helps them understand family dynamics and the impact of psychosocial issues and problems on the veteran's health and response to treatment. Conducts formal workshops and classes for staff. He/she is accurate, literate, and timely in charting, correspondence, and reports. Incumbent judges effectiveness of performance and when indicated seeks consultation from supervisor, cliriic manager or Social Work Chief. The incumbent maintains records and statistics required by the assignment, as well as any other information which may be needed for special studies and reports. Participates in staff development and training activities, research, program planning and development. Assumes responsibility for professional development by various venues such as: education of current theory and treatment modalities, reading professional journals, attending conferences, clinic seminars and teleconferences, and utilizing materials provided by VAMC. Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, the Joint Commission standards and facility standards Demonstrates knowledge and skill in the use of software applications for drafting documents and data management as well as other computer systems in use by the VHA. Collaborates in the performance improvement processes and complies with performance measures. Serves on committees, work groups, and task forces at the facility as deemed appropriate by immediate supervisor and Social Work Chief. Reports directly to the Coordinator of the Medicine Social Work Service but practices independently, exercising initiative and independent judgment in day-to-day activities. Supervision/consultation is of an administrative and professional nature. In unusual and/or complex administrative and clinical problems, the incumbent initiates appropriate consultation with Supervisor, Chief of Social Work as well as with other authoritative resources at the Medical Center.
