Patient Service Representative - PART TIME Home Health
GENERAL DESCRIPTION OF THE JOB CLASS Plan, coordinate and assist the Home Health team or program in the establishment and operation of a specialized program and/or activity at Duke HomeCare & Hospice, planning methods and developing systems in accordance with predetermined goals and objectives.
DUTIES AND RESPONSIBILITIES OF THIS LEVEL
Department supervisor/manager Major Job Functions:
Implement appropriate information systems and work processes in support of home health activities.
Inputs visits and maintains clinician visit schedules within IT scheduling system. Collaborates with supervisor and updates schedules based on physician orders, clinician competency and availability. Must be knowledgeable of home health regulations related to skilled needs.
Works directly with referral center. Coordinates with Supervisors/Manager throughout the day as to availability for admissions.
Assigns visits to clinicians and distributes patient information to clinician accordingly. Visits include admission visits, visits related to change in physician orders and coverage visits.
Contacts patient or caregiver prior to initial home visit to confirm start of care and address, and to determine involvement of other agencies or home bound status if possible.
Inputs all visits that are documented on paper, (primarily per diem and contract staff).
Identifies potential visit inconsistencies related to length of time or frequency. Updates clinical information system as necessary.
Updates system advisories as assigned.
Run utilization reports related to visits, missed visits and documentation submission. Identifies
