VA Lebanon Healthcare System is seeking a full time, permanent Physician Assistant (Hospice-Palliative Care) aligned under the Medical Care line. You can be part of that team, helping to make sure that our Veterans continue to receive the highest quality care possible. The Physician Assistant (Palliative Care) works in collaboration with and with available guidance and supervision of an Attending Physician(s) in the field of Palliative Care. The primary practice site for Palliative Care is in the Palliative Care Clinic in Building 17 located on the main campus of the Lebanon VA Medical center in Lebanon, Pennsylvania. In addition virtual care consults are available with each of the five Community Based Outpatient Clinics and for outpatient visits where clinically appropriate. Additional duties may also include follow up via electronic media. Incumbent will be responsible for Palliative Care duties which are generally defined as but not limited to: 1) Provides Inpatient and Outpatient Palliative Care consultations and clinic appointments for Veterans with both acute and routine Palliative Care concerns via various modalities of care. 2) Performs comprehensive history and physical examinations and initiates treatment when indicated for acute and chronic medical conditions. 3) Works with a multidisciplinary team that includes nursing, social work, dietetics, clinical pharmacy, physical therapist, behavioral health and clerical support. This also includes weekly Team meetings. 4) Orders diagnostic tests, medication, laboratory tests as medically relevant for each Veteran. 5) Diagnosing medical conditions predominantly associated with the field of Palliative Care. 6) Inpatient consults for Goals of Care. Support palliative care nurse with orders/symptom management needs from phone calls to clinic. 7) Assist with orders/family meetings for those admitted to 1-3A for palliative needs. 8) Assist with managing calls to the narcotic line ext. 9) Completion of FMLA forms for family members who need to assist with care needs. 10) Manage/track urine drug screens and Narcan prescriptions for veterans prescribed opioids. 11) Review and update reconciliation of medications. 12) Evaluates, consults, develops diagnoses, and manages the panel of Veterans assigned. 13) Determines medical assessment and develops treatment plans according to accepted protocols. 14) Future endeavors could also include additional education of primary care providers and further expansion of telemedicine to other sites. 15) In collaboration with a supervising physician, the PA is also authorized to write orders on outpatients acquiring their care at the Lebanon VA medical center. This may also include inpatient consultations on patients admitted to either the medical or surgical services in the acute care hospital at the LVAMC. 16) Ensures practice is consistent with VA policies and procedures, clinical emergency medicine guidelines, HIPPA policies. 17) Uses prescriptive authority granted by licensure. Work Schedule: Typical tour of duty is expected to be Monday - Friday, 8:00am - 4:30pm; however, weekends and nights may be required if Veteran needs should arise.
