Learn more The Northern California health Care System is currently recruiting for a Senior Social Worker Community Living Center (CLC) for Martinez location. This position is in the Geriatric/Extended Care Service Line. The CLC senior social worker is responsible for advance level case management and coordination of care for patients across the continuum of care, including initial assessments, treatment plans, VA & community referrals, Hospice/Palliative care, supportive counseling, grief/bereavement for patient's families/caregivers, and discharge planning. Will have high level knowledge of Veteran's benefits and services relating to special programs, VA funeral & burial services, service-connected compensation and non-service-connected pension. Will participate in MDS completion as required for every CLC patient. The senior social worker possesses the skills to offer psychosocial interventions and measures outcomes to interventions. Must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families, and groups. Must be able to facilitate difficult discharges that require MediCal placements, conservatorship, etc. Major duties include, but are not limited to: Participate in the development, evaluation, and communication of standards, policies, and procedures, and plans of care for the CLC. Educate medical center colleagues and/or students regarding the various GEC programs and services and assist with dissemination of information to other VA facilities and community partners. Evaluate CLC care practices using relevant VA statutes and VHA regulations and take action to improve identified deficiencies. Track CLC care program assessments, consults, authorizations, statistics, data management, identify significant findings, problem solve, complete documentation reports pertaining to caseloads, productivity, and trainings. Complete an in person initial screening and evaluation of Veterans to determine their needs and if they meet criteria for inclusion in various GEC programs in order to maximize function and independent living to the extent possible. Develop a treatment plan of the Veteran in collaboration with the referring and CLC interdisciplinaryteam, the Veteran, family members, and caregivers, whenever possible. Serve as a liaison between the CLC Team and specialists and Veteran and/or family. Review services which may be used to provide in home services to Veterans. Make rapid assessments and develop crisis management plans to maintain a patient in the home or for admission to acute, short term, and/or long-term placements. Provides education related to VA and community resources, entitlements, Advance Directives/Living Will, Life-Sustaining Treatment decisions, and will refer Veterans and/or families to other GEC programs or appropriate VA resource for identified health education needs. Coordinate referrals to non-institutional services in collaboration with team members. Provide Veterans and their caregiver's ongoing supportive counseling.
