The Licensed Practical Nurse at the VA Puget Sound Health Care System in Seattle, Washington provides patient care in accordance with established clinical nursing practices under the direction of an RN or physician The inpatient LPN may be assigned to one of the following units: 6 West is a 30 bed inpatient unit that provide medical care and specializes in cardiac and neurology care. 7 West is a short stay psychiatric unit with both voluntary and involuntary admissions. Patient populations include Veterans who are likely to present with acute psychosis, bipolar affective disorder, major depression, schizophrenia and suicide/homicide ideation with plan. Some patients may present with concomitant substance use intoxication that may require detoxification while being treated for their presenting psychiatric disorder. The Community Living Center provides inpatient Veteran/resident care for Veterans with acute and chronic health conditions with diverse culture and religious/gender orientation. Veterans may need dementia, rehabilitation, long term, palliative and/or hospice care. DOM - The Mental Health Residential Rehabilitation Program facility, also known as the Domiciliary, is a 64-bed residential treatment facility that provides treatment and rehabilitation for male and female Veterans suffering from PTSD, homelessness and substance abuse. Of these, 20 beds are specifically allocated to Veterans dealing with military related PTSD, including military sexual trauma LPN Duties include but are not limited to: Delivers care to patients in a respectful non-judgmental manner and according to Patient Care Plan Demonstrates understanding of age - and population - specific characteristics that affect the health care needs of the veteran. Performs aspects of admission /discharge procedures as directed Collects patient data including observations of patient status (i.e. vital signs, intake/output, skin condition, mental status, etc.). Accurately reports patient/client information. Recognizes subtle and significant changes in the patient in response to nursing care. Promptly recognizes urgent or emergent patient care situations, seeks assistance and effectively intervenes as directed when appropriate. Communicates changes in patient condition/response to nursing care. Organizes/ prioritizes routine and non-routine nursing care activities (ADLs, medication administration, wound care, EKG, foley catheter insertion, bladder scan, etc.) Provides care to patients/clients appropriate to their cognitive, physical, emotional and chronological maturation. Accurately documents and retrieves patient/clinical information using electronic medical record. Provides effective education to patients and/or family members in relation to common disease processes, medications, and/or prescribed diagnostic and/or treatment regimes, as directed. Adheres to standards of practice and safe technique in preparation and administration of medicine, documents administration in BCMA. Observes patient for adverse effects from medication and reports to the team RN. Work Schedule: Inpatient Acute Care ( 5 East, ) is 24-hour units, shifts variable, to be determined by manager upon hire, based on needs of the unit. CLC American Lake: 24-hour units, Day Shift 0600-1430, Evening shift 1400-2230, and Night Shift 2200-0630, to be determined by managers upon hire, based on needs of the unit. CLC Seattle: 24-hour units, Day Shift 0730-1600, Evening shift 1530-0000, and Night Shift 0000-0800, to be determined by managers upon hire, based on needs of the unit. Inpatient Mental Health (7 West) is 24-hour units, shifts variable, to be determined by manager upon hire, based on needs of the unit.
