About the Position:
Reporting to the Director, Patient Care Services, the Associate Director, Patient Care Services is an executive-level position responsible for the implementation, supervision, and oversight of all patient care programs and achievement of both clinical and administrative outcomes.
This position requires excellent verbal and written communication skills; the ability to gain the trust, respect, and support of the nursing staff; and effectiveness in building relationships with hospice executive staff, physicians, the entire interdisciplinary team, and partnering providers in the community. The position requires superior flexibility in both tasks and responsibilities as the role grows and develops with the company.
The successful candidate is someone who shows up, is kind, tells the truth, keeps their word, and turns ‘complaints’ into ‘requests’. LightBridge is seeking a motivated, hardworking, caring, compassionate, and loving nurse with hospice experience.
Short Term Goal (First 6 months):
Prepare for the 2021 Joint Commission (TJC) survey (expected August-November).
Long Term Goals (6+ months):
Provide clinical oversight to existing and new programs.
Embrace new product lines and serve as a change agent when implemented.
Nurture your team’s development by embracing change, promoting growth and development, championing new programs, and serving as an intuitive and caring leader.

Minimum Qualifications – Must Meet 1 of 3:
CA Registered Nurse with a BS or MS in nursing or another health-related field, with 3 years of experience within the last 5 years in hospice or home health agency, or primary care clinical or health facility, at least 1 year of which was in a supervisory or administrative capacity.
CA Registered Nurse with 4 years’ experience within the last 5 years in hospice, home health agency, hospital ICU, or primary care clinic or health facility, at least 1 year of which was in a supervisory or administrative capacity.
CA Registered Nurse employed as the Director of Patient Care Services in a hospice program on the effective date of the 1993 CHSA Standards of Quality Hospice Care.
Responsible for the planning, coordination, and delivery of quality interdisciplinary Hospice and Palliative care services.
Directs all patient care services for the team, ensuring services are rendered as integrated components of the interdisciplinary plan of care.
Collaborates with both the medical director and the attending physician regarding palliative care measures for pain and symptom management.
Assures complete and accurate data and documentation are available to make decisions and further assures that those decisions are made on a timely basis.
As necessary, reviews information on all pending and new admissions and discharges to assure that eligibility and appropriateness criteria are met.
Monitors the type and level of team services provided (staffing, medications, HME, supplies, level of care changes to inpatient and continuous care, etc.) to assure they are cost effective, appropriate, and meet the needs and expectations of the patient/family.
Manages the team schedule and assures coordination of services 24 hours a day, 7 days a week.
Participates in team meetings, assuring interdisciplinary input into the update/revision of the plan of care in concert with the changing needs and expectations of the patient and family. Participates and leads the nursing meetings as appropriate.
Provides immediate follow up and investigation for all hospitalizations. Gathers facts and evaluates situation with DPCS and others, as necessary.
During periods of staff shortages, willing to provide patient care, including evaluating and admitting patients, as necessary.
Participates in the administrator on call rotation for the organization, assuring 24/7 oversight of patient care.
