Advanced Practice Registered Nurses / Physician Assistants
RWJBH is seeking a compassionate, knowledgeable and collaborative Nurse Practitioner to work full-time with our long-established Inpatient Palliative Care Program. This is a day position working 40 hours/week.
Coordinate and manage inpatient hospital consultations to the palliative care service and physician, staff and community education..
Plan, attend and facilitate family meetings for goals of care and advance care planning. Perform completion of advance directives and POLST forms with patients.
Routinely collaborate with attending medical team to discuss and establish palliative care plan as it relates to their overall clinical condition.
Frequent communication and coordination with other members of the healthcare team to ensure the best patient outcomes.
Document clinical notes timely to communicate the palliative care plan as well as support appropriate and timely billing of palliative care services.
Educate all members of the healthcare team on a case by case basis as well as in educational forums/seminars/presentations, including but not limited to ELNEC, Grand Rounds, and staff orientation.
Educate the community, including lay persons as well as professionals in healthcare facilities, on the tenets of palliative care, pain and symptom management, advance care planning and POLST forms.
Weekly participation in palliative care interdisciplinary meetings and daily morning huddles.
Participate in on call coverage for the palliative care service.
Participate in biomedical ethical discussions and consultations as needed.
Participate in the development and revision of policies and standards, including but not limited to Joint Commission Certification.
Maintain Advanced Certification in Hospice and Palliative Nursing (ACHPN) or obtain and maintain certification within one year of hire.
Perform and participate in research as necessary to advance the specialty of palliative care.
Education and Skills Required:
Masters in Nursing as a Nurse Practitioner, DNP preferred.
Graduate level palliative care training preferred.
Specialty level certification preferred. Mandatory to be obtained within two years of hire and maintained throughout employment.
1-3 years palliative care experience preferred or experience with complex disease management and advance care planning.
Excellent communication and interpersonal skills, both verbal and written.
