This position serves as a liaison between Hospice and the Community, Case Managers, Physicians, Patients and Families to ensure continuity of quality care to the hospice patient. This position facilitates & assists in the day-to-day operations of the intake process and to be accountable for a smooth admission throughout a patient's/family's transition from admission to the Interdisciplinary Care team. This position will assure prompt and appropriate delivery of patient care in accordance with physician orders and State & Federal regulations.
