Both our VNA Palliative Care and VNA Hospice departments are seeking licensed RNs knowledgeable in community health nursing to provide nursing care to patients in the community setting. Responsibilities include conducting scheduled home visits according to client acuity and in accordance with the organization's emergency preparedness plan; performing physical and psycho social assessment and preparing documentation for each admission to caseload; and developing an individualized and holistic plan of care for the skilled care of the patient in collaboration with the patient/family/caregiver.
Graduation from an NLN-accredited school of nursing; BSN preferred
Massachusetts RN licensure
Advanced clinical assessment skills with ability to deal with a variety of clinical situations
1-2 years of nursing experience preferred
Experience as a Community Health or Hospice nurse preferred; Med/Surg a plus
