We are looking for an LPN to serve patients in their home setting for home care visits in the twin cities. Our LPN's provide services such as; drain/tube cares, wound care, draw labs, tube feeding, obtain MD, flush central lines, IV fluids, wound vac, trach cares,and lvad dressing under the supervision of a qualified registered nurse.This position involves travel in the metro area. Home office will be in Maplewood, MN
Work schedule is weekdays, 8:00AM - 5:00 PM and includes a weekend and holiday rotation.
The Home Care LPN provides skilled nursing care to home care patients according to plan of care, including advanced nursing interventions related to patients needs. Provides services under the supervision of a qualified registered nurse.Reports pertinent observations to the Case Manager/RN.
Consults with the Nurse Case Manager as needed to make modifications in the plan of care, and obtains physician orders for all changes made in the plan of care. (Physician orders must be discussed with the RN Case Manager).
Counsels the home care patient, family and caregiver regarding care needs and health promotion/prevention and reinforce teaching/education at the direction of RN.
Monitors the patients progress and responses to care/services delivered, and update the Nurse Case Manager, physician and other health care team members as indicated.
Maintains complete, accurate and timely clinical records/documentation on all patients according to agency policy.
Stays current (expert) in home care regulation and all applicable licensing.
Actively participates in educational programs, meetings and agency committees as appropriate.
Accepts responsibility for the understanding and implementation of the discipline applicable content of all departmental communications and procedures.
Identifies processes to improve and potential solutions.
Licensed as an LPN and eligible to practice in Minnesota
Current CPR (BLS for Health Care Providers) certification. From either the American Red Cross or American Heart Association
Six months recent experience in a med/surg, long term care, or home care/hospice setting as a CNA, HHA or LPN.
Two or more years recent LPN experience in a med/surg or home care/hospice setting.
Valid Minnesota drivers license (or license that matches your current permanent residence)
Current and adequate automobile insurance policy
Computer skills (able to document effectively in EMR)
