Our specialized Palliative Care Program focuses on providing relief from the symptoms and stress of a serious illness. The goal is to improve the quality of life for both the patient and the family by working with the patient’s primary care physician (PCP) to provide an extra layer of support in the home or hospital. Palliative care is appropriate and at any stage in a life-limiting illness and, unlike hospice care, can be provided along with aggressive treatment. We are the eyes and ears for the PCP and specialists, assessing and reporting patient issues as they arise or change.
Job responsibilities include:
Disease education for patients and families
Symptom assessment and management
Ordering of medications, treatments and diagnostic tests to manage chronic conditions effectively
Prevention of unnecessary hospitalizations, and promote patient comfort/wishes
Facilitation of patient transfer to hospice care, when appropriate
Must be licensed to practice in state where the Palliative Medical Associates is located.
Certified as APRN/(ARNP)/CRNP.
Master's of Science in Nursing - MSN is required.
Qualified candidates must have a minimum of five (5) years nursing experience.
Clinical nursing care experience in oncology, hospice, geriatrics, intensive care is preferred.
ANCC or AANP certification mandatory. (ACHPN certification considered a plus, but is not required.)
Palliative care APRN preferred.
Knowledge of the interdisciplinary team concept.
Reliable transportation, current state driver’s license and automobile insurance.
