Haven cares for over 90 percent of patients in their home or wherever they call home. But when there is a need for short-term inpatient care, Haven offers hospice care centers in Gainesville, Palatka, Chiefland, Lake City and Orange Park. In these welcoming and home-like settings, Haven clinicians provide around-the-clock care to patients needing intense pain and symptom management as well as provide education and support to family and loved ones. The care centers offer overnight, respite care to provide a break to primary caregivers for up to five days.

Associate Medical Director (Lake City & Chiefland): As an Associate Medical Director, at Haven you will have the opportunity to make an impact on the lives of our patients and their families on a daily basis. You will work with an Interdisciplinary team to help develop and review patient care plans. You will serve as a liaison and contact between the Haven program, community physicians and others engaged in health care services in order to meet the needs of the patients and families served by Haven. In coordination with the Chief Medical Officer, you will become a member of the medical team which provides physician oversight of the patients twenty-four (24) hours a day, seven (7) days a week. This includes joining the on-call rotation and some weekends.
Access Associate Medical Director: As the Haven Access Associate Medical Director, you will work closely with the Access department, which triages incoming calls and referrals. The Access Physician reviews incoming referrals for hospice eligibility. You will provide physician direction and education to admissions personnel, assist with reviewing records, and speaking with referring physicians, when required, in order to establish eligibility. You will complete a certificate of terminal illness narrative for each hospice-eligible patient. The Access Physician can work from anywhere in the Haven service area – including working from home. You will be required to report to the office one day per week and cover on-call and some weekends as required.
Palliative Care Associate Medical Director: Haven recognizes that all hospice care is palliative care, but all palliative care is not hospice care. As the Haven Palliative Care Associate Medical Director, you will have the opportunity to make an impact on the lives of our patients and their families on a daily basis. You will work with a team of providers to treat patients’ pain and symptoms in both inpatient and community-based settings. You will develop strong relationships with community providers by coordinating your care with the care of patients’ primary care physicians and other specialists. You will also train other Haven providers in palliative care and, in conjunction with the Director of Palliative Care, supervise palliative services.
The Palliative Care Associate Medical Director can work from any Haven location but must be able to see patients in all of Haven’s service areas.
