This position supports Holy Cross Health, which includes Holy Cross Hospital, Holy Cross Germantown Hospital, and Holy Cross Health Network. Employees have a primary work site for supporting the health system. At times, the site may be subject to change, depending on the needs of the organization.
The Palliative Care Nurse Practitioner collaborates with the Palliative Care Medical Director and is a member of the Palliative Care team. As a member of the team, performs and documents assessments and consultations on patients, provides staff and patient education and works with the team in developing the Palliative Care Program. Functions within the standards, policies, procedures and guidelines for the organization.
Supports the Mission of Trinity Health and Holy Cross Health.
Licensed as a Registered Nurse in State of MD or other compact state Licensed as Nurse Practitioner in State of MD Current DEA License BLS
Minimum Knowledge, Skills, & Abilities Required:
Minimum of 3 years experience in nursing. A minimum of 2 years experience in Pain Management, hospice or palliative care preferred.
Possesses effective verbal and written communication skills.
Displays strong interpersonal skills.
EPEC (Education in Palliative and End of Life Care) and ELNEC (End of Life Nursing Education Consortium) training preferred or completion of program with similar content/competencies
Must have visual and auditory acuity.
Uses computer and automated technology appropriately
Ability to independently perform Palliative Care consultations and develop care plan consistent with patient and family goals.
Experience with conducting family meetings and discussing goals of care,
Understand concept of comfort care and have ability to transition patients to a comfort care plan, including palliative ventilator extubation.
Training and experience in managing pain, prescribing opioids (all routes- oral/ subcutaneous/ intravenous/ transdermal/ transmucosal/ rectal), PCA (Patient Controlled Analgesia) pumps, rapid opioid titration and managing non-pain symptoms.
Consult with palliative care physician or designees as needed, inform primary physician of services provided and collaborate with other physicians as needed.
Assist in all facets of care coordination for palliative care referrals.
Prepare and maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
Assist in development of clinical practice guidelines/standards in support of quality palliative care.

Physical Requirements: Exerting up to 35 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects.
Visual Acuity: The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection); using measurement devices; and/or assembly or fabrication parts at distances close to the eyes.
Environmental Conditions: Significant amount of time spent on nursing units and in other areas of the hospital interacting with staff, physicians, and patients/families, as necessary.
Reports to the Medical Director, Palliative Care.
