Hospice of the Piedmont is seeking a full-time Hospice Liaison to increase awareness of services provided, provide organizational customer support, through outreach to facilities, physician offices and other medical organizations by developing integral relationships throughout the community. The Hospice Liaison will communicate with referral sources and assist with questions, concerns or requests related to Hospice of the Piedmont and demonstrate a thorough and complete knowledge of Hospice of the Piedmont’s mission, vision and values.
The Hospice Liaison duties include but are not limited to, prepare and execute an extensive outreach plan to develop, establish and maintain strong relationships in our community while increasing overall admissions; develop and maintain a database of current and potential referral sources and monitor utilization of Hospice of the Piedmont to include trends; identify service opportunities and work in conjunction with team members to provide quality care; address specific patient needs; and serve as a representative at area facility and medical organizational events.
The Hospice Liaison position requires a Bachelor’s Degree in a related field; licensed as Registered Nurse or Social Worker in the Commonwealth of Virginia preferred, as well as previous experience. Must have excellent leadership and organizational management skills as well as excellent verbal and written communication skills; and the ability to take the lead role in developing, implementing and executing the program.
