The professional home health nurse will assess human responses to disease process, illness and/or injury. The registered nurse will identify health problems and provide restorative, supportive, and preventive nursing measures to individuals in their home by executing the medical regimen prescribed by a licensed physician.
The registered nurse is fully responsible for all actions as a licensed nurse and is accountable to clients for the quality of care delivered. The registered nurse reports to his/her designated immediate supervisor regarding work planning and assignments.
Reports directly to the Home Care Nurse Manager.
COMPETENCIES AND SKILLS:
Must possess strong interpersonal, oral and written communication skills.
Self-starter with strong organizational and time management skills. Detailed oriented, extremely well organized and able to plan and problem solve.
EDUCATION AND/OR EXPERIENCE:
Registered Nurse with valid Pennsylvania licensure to practice required. Bachelors Degree preferred.
Specialized knowledge in HomeCare & Hospice regulations, eligibility criteria for home bound status, and an interest in care in the home setting preferred.
A minimum of two years experience and interest in working within a home setting required. Community Health experience preferred.
Computer literacy required with specific experience in Microsoft applications and Windows operating platform required.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Must be available to work in a rotating basis. This position involves occupational
exposure to disease and illness including, but not limited to Hepatitis B and HIV viruses.
