Corizon Health, the nations leading correctional healthcare organization seeks a Hospice Manager to run their Care Choices Program at the Michigan Department of Corrections. Work at the Michigan Regional Office in Lansing, MI and Duane Waters Health Center, an Inpatient Hospital in Jackson, MI. Some travel to facilities in Michigan will be required.
The Hospice Manager is responsible for participating in health system strategies and planning efforts to advance palliative care/End of Life services within the CHOICES program. Evaluating the necessity, appropriateness and efficiency of the use of medical services, procedures and facilities for each patient. Responsible for clinical review of all CHOICES patients and services for appropriateness based on medical criteria, the management of healthcare resources necessary and appropriate for achievement of desired outcomes, and the coordination of alternative levels of care for members. Serves as a patient advocate, seeking and coordinating creative solutions to patients health care needs without compromising quality outcomes.
*Participating in health system strategies and planning efforts to advance palliative care/End of Life services within the CHOICES program. *Through clinical review, initiate and facilitate patient and family centered goals of care discussions including those focused on end of life. Counsel and advocate for patient and family by providing relevant information to clarify options of care. *Collaborate with multidisciplinary teams to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures. *Seeks ways of reducing outside services costs and improving utilization as well as acting as a liaison between Corizon and off-site providers. *Responsible for ensuring the daily and monthly reports are sent to the appropriate sites/personnel and participates in Utilization Review and others as required. *Provides assistance in the development and implementation of Utilization Management Policies and Procedures for the CHOICES program and to continually seek inventive methods for reducing outside services costs. *Assists with the hiring, training, developing and evaluation of staff within the CHOICES program. *Liaisons between Corporate UM, on-site physician staff and off-site hospital staff. *Complies with all rules, directives, policies, procedures and protocols of Department of Corrections and Corizon Health. *Assumes additional duties as necessary. *Contribute to Mission effectiveness by promoting the spirit embodied within the Mission Statement.

Must be able to handle multiple tasks, be able to work in a stressful environment and take appropriate actions as needed. Demonstrates proficiency in Microsoft Word, PowerPoint, and Excel. Must follow departmental policies and procedures at all times.

*Registered Nurse within the State of Michigan *2-3 years of related experience. Certified Hospice Palliative Care Nurse (CHPN) or within 12 months of hire. *Licensure / Certification / Registration Requirements: Completes 12 CEU credits per year and maintains active state licensure.
