Palliative Care Nurse Practitioner Opportunity in Northern CA Wine Country!

St. Joseph Heritage Healthcare has an opportunity for an experienced Palliative Care Nurse Practitioner. This position will function independently as an advanced nursing professional in palliative care across the health care continuum under direction of the palliative medicine physicians and administrative guidance. Assesses the patient to identify palliative care needs; creates and modifies a plan of care; makes appropriate referrals; provides patient and family education; monitors patient status; and evaluates the effectiveness of the established care plan. Serves as an extension of the palliative medicine physician and a resource for medical and nursing staff and works collaboratively to develop and implement palliative care program across all inpatient and outpatient settings. Demonstrates a high degree of clinical expertise in providing care for complex palliative care patients and their families. This position is a combination of inpatient and outpatient care.
Qualifications: Education: Graduated from an accredited school of nursing and an accredited Nurse Practitioner Training Program. Experience: Prefer three or more years of Acute Nurse Practitioner experience, working in Palliative Care.
License Certification Requirements: • Graduate from an accredited Nurse Practitioner training program • Board of Registered Nursing Certification of Nurse Practitioners – Acute Care Nurse Practitioner • Advanced Practice Level Certification • Board of Registered Nurses Certification Furnishing Number • Current BLS certificate • Current RN license • DEA License
Preferred Position Qualifications: National certification within specialty (Advanced Practice Hospice or Palliative Nurse (ACHPN)
