Advanced Practice Registered Nurses / Physician Assistants
We are currently seeking nurse practitioners to join the Palliative Care and/or Hospice teams at UNC Health Care. The nurse practitioner in collaboration with a physician will provide comprehensive, quality care to patients and families with serious or terminal illness.
The scope of work may focus solely on palliative care, or hospice care; or may be a blended role of palliative care and hospice care. For palliative care the location of work may range from inpatient, outpatient to community based. Hospice care will be a blend of inpatient care at the UNC Hospice House or hospice care in the home. The Palliative Care and/or Hospice schedule will be primarily days with on-call, weekends and holidays. Compensation and benefits are competitive
JOB DUTIES AND RESPONSIBILITIES
Proactive identification of opportunities to innovate in care delivery in order to improve the patient and caregiver experience of care for serious illnesses
Provision of direct patient care and consultation of patients referred for palliative care or hospice consults and/or service
Assistance with diagnosis, treatment and management of acute and chronic health conditions within scope of practice
Medication reconciliation and optimization, including discontinuation of non-beneficial medications when appropriate
Creation of patient plans of care which may include course of treatment for patient, ensures continuity of care and plans for discharge.
Evaluation and treatment of emergent and ongoing physical, emotional, social and spiritual symptoms and determine executive, cognitive and functional capacity of patient and caregiver
Collaboration with interdisciplinary teams including home health nurses, hospital nurses, social workers, attending physicians, palliative care or hospice physicians and others to identify patient needs and order medication adjustments, treatments and other care
Building effective relationships with other health care providers and referral sources to ensure excellent team follow-up
Developing curricular materials and participating in the education of other medical professionals
Completion of an accredited graduate-level Advanced Practice Provider – APRN program in good standing
Licenses as Nurse Practitioner in the state of North Carolina
